Need healthcare enrollment assistance? Platoon Ready to Help

Every Tuesday from 12:30 - 4 pm.

A team of U.S. veterans, hired by NJ Hospital Association, have been trained as Certified Application Counselors. They are prepared to assist folks in comparing and signing up for new health insurance plans through the Health Insurance Marketplace, Medicaid and NJ Family Care.

Rotations of CACs will be available to assist folks on a drop-in basis (no appointments).

WHEN:             Every Tuesday from 12:30 - 4 pm
WHERE:           CentraState Medical Center, 901 West Main Street, Freehold, NJ 07728
SERVICES:       Education, Enrollment Assistance, Spanish language assistance available
MORE INFO:     N.J. Townes, 609-480-8834, ntownes@njha.com (Monmouth team lead)

Additional locations being added, please check www.njha.com/reform/find-a-navigator/ for latest schedule.

Some will qualify for tax credits or subsidies that will make their coverage more affordable. To help the CACs, come prepared with information (for each household member needing insurance) such as:
•    Social Security numbers (or document numbers for legal immigrants)
•    Birthdates
•    Pay stubs, W-2 forms or “Wage and Tax Statements”
•    Policy numbers for any current health insurance
•    Information about any health insurance available through your job, if you are currently employed.

This service is made available through a special initiative from the New Jersey Hospital Association and New Jersey Health Initiatives, with financial support provided by a grant from the Robert Wood Johnson Foundation.

The CACs will be working in hospitals and other sites across the state. Visit www.njha.com/reform/gethelp to learn more about this program and see the CACs’ full schedule throughout NJ.
